CARE UNIT

The palliative care
Unit is part of the Bluewater Health Palliative Care Program. The
Unit exists to serve the Sarnia-Lambton region and functions in close
cooperation with all existing agencies.

How to Obtain the
Services of the Palliative Care Unit:
| For admission to the Palliative Care
Unit, the patient must be registered with the Palliative Care Program
located at the Lambton Hospitals Group-Norman Street Site. Admission to
Palliative Care is temporary. Once a patient’s condition has
stabilized, his/her care needs will be reassessed to determine if they
can be at home or if long-term care is needed in another setting. |
Admission

such as the patient, family, clergyman, etc. All requests made in
this way will be discussed with the family physician prior to a
visit and assessment

HOSPICE
The Hospice is a
home, which provides:
-
Compassionate, supportive care to
individuals and their families.
-
Skilled supervision, personal
assistance and meals for people with a terminal illness who are too
frail to live alone and do not need to be in hospital.
-
A non-profit community based service.
-
An outreach of the
Bluewater Health, Palliative Care Program.

are made:

Individuals, family
members, friends, physicians, clergy, social agencies or any health care
professional can request application information by calling the Hospice
Office at 519-464-4400 ext. 8499.

PALLIATIVE
CARE OUTPATIENT CLINIC
|
Out Patient
Clinic Services:
-
Pain Management
-
Management of other symptoms
-
Emotional support of patient and family
-
Introduction of all palliative care
services in the community
-
Education (i.e. disease process,
medication and treatments offered).
While most patients
referred to this clinic have cancer, other diseases that are life
threatening may benefit from this service. (E.g. Chronic Lung Disease,
ALS, Heart Failure, End-Stage Kidney).

Clinic Staff:
Palliative Care
Physician-This is a physician who works regularly on the Palliative Care
Unit.
Clinical Resource
Nurse-This is a full time registered nurse (RN) from the Palliative Care
Unit. Other team members may see the patient in the clinic if required.
(E.g. dietician, social worker, pharmacist or community care access
centre case manager.
The Palliative Care
Clinic works with:
-
Family physician who receives an
initial consultation note and a copy of the ambulatory care chart
for every visit.
-
Palliative Care Unit staff that
have access to the clinic charts and may be able to address
issues/problems. Clinic patients are encouraged to contact the
Palliative Care Unit with any concerns. If necessary, the Palliative
Care Unit clinical resource nurse will contact the physician. The
Palliative Care Unit secretary arranges the appointments and phones
to remind the patient prior to the appointment date.

Community Care Access Centre
(CCAC) who will arrange home care services if required. The visiting
nurses (Comcare, We Care, VON) are able to communicate with both
physician and clinical resource nurses regarding patient care.

Local Oncologist, Local Cancer
Clinic and/or The London Regional Cancer Centre if they are
involved.
