The
Palliative Care, Pain &
Symptom Management Program, Lambton County welcomes you to
this site. The Program
works towards ensuring that persons with advanced disease or terminal
illness and their families have access to health care providers who have
expertise in the management of pain and other distressful symptoms.
Program
Origin
In December 1992, the
Ministry of Health announced funding for 4 initiatives designed to
enhance the care of those with life threatening or advanced disease.
Program
Focus
When a person is
facing a life-threatening or advanced illness, a number of physical,
emotional, and spiritual concerns may interfere with overall comfort and
quality of life.
The
Pain and Symptom Management Program is dedicated to help relieve such
concerns by assisting individuals to access palliative care and by
providing education and information to health care providers.
User-Friendly
Access
Anyone desiring
information about palliative care services are encouraged to call the
Program Consultant. This may include health care providers from area
community agencies, hospitals and long-term care facilities, as well as
individuals or family members that are dealing with life-threatening or
advanced illness.
Services
Offered
The
Palliative Pain and Symptom Management Program offers a number a number
of services provided by the Palliative Pain and Symptom Management
Consultant/Educator (PPSCE) such as:

Consultation
concerning pain and symptom management to health care providers in the
community, long-term care facilities, hospitals, and to patients
with life-threatening illness.

Planning
assistance to health care facilities and agencies in the development
of their palliative care teams or services.

Educational
opportunities for health care providers:
Hospice Palliative Care - The fundamentals,
Comprehensive Advanced Palliative Care Education, Advanced
Palliative Care for Support Workers, Refresher Days, Mentorship
Programs.

Educational
materials concerning pain and symptom management, palliative care.

Assistance
to individuals to access palliative care services.

Links
to Palliative Pain and Symptom Management Programs in other
communities.

Volunteer
Palliative Care Visiting Program is offered through the Sarnia
Victorian Order of Nurses.
